I just went outside to look at my gas cap. The screws are flatheaded , countersunk ( obviously ) sheetmetal screws, and have a Phillips ( crosspoint ) head. The "special" bolt, which should be obvious where it goes, is, I'm sure, there as a security measure. Original flip open gas caps have that "special" bolt as well.
 
The little divot out of the washer is likely a stamping error. The washers are stamped out of a large metal sheet that travels under the stamps. The metal sheet didn't advance far enough and the next row that was stamped out was too close to the previous row. I have encountered this numerous times. If you go to a hardware store that sells in bulk and dig through a washer bin you'll likely find some.
