1973 Mustang Sportsroof. Currently a work in progress!!!
I now understand why people don't replace the parts, but only attach an after-market fix.  This is a PITA.  Especially with the trans still in place.

What is the best way to remove the nut holding lever on?

I'm seriously considering just replacing the ratcheting shift component.

I modified an end wrench so it could get into it.
I have a set of open wrenches for a ratchet, but the don't fit in to the space.  The nut is 3/4 or larger.

 I may try a pair of needle nose vice grips and then try turning the outer end with vice grips.

 
Here are the pictures of my modified wrench. The end for the nut on the shift lever is on the right. I started with a 3/4" wrench, but had to widen it out to fit the nut. It measures slightly less than 7/8".
